Physical Edge Ltd is a Healthcare Company providing Physical Therapy services for musculoskeletal injuries and operations. The range of treatments you can expect at Physical Edge Ltd includes; Physiotherapy, Sports Therapy, Sports Massage, Personal Training, Acupuncture, Pilates and Yoga.

Physical Edge Ltd takes your privacy seriously and will only use your Personal Data for matters related to your Physical Therapy Treatment. The Personal Data we will collect from you is full name, home address, date of birth, contact phone numbers, email addresses and referral source. You will be asked for this information on your initial appointment and it will be stored on a cloud based secure medical software system. Any paper copy of this information will then be disposed of by shredding.

The lawful basis we use for processing your personal data is Legitimate Interest. We require your Personal Data to identify you, contact you, and save your medical records on our secure medical software system. This Data is also required for communicating with insurance companies, and other medical professionals, therapists, exercise specialists or others for the purpose of contributing to the greater overall management of your medical care. Any shared communication will require your written consent. If communication of your Personal Data is required outside of the European Union, you will receive an encrypted email with this information to give to the person/ company of your choice.

Your Personal Data will be retained for medicolegal reasons as recommended by the Chartered Society of Physiotherapy; for Children and Young people until their 25th birthday or 26th birthday if 17 years old at the conclusion of treatment or 8 years after death, for Maternity records 25 years after the birth of the last child, for Mentally disordered person 20 years after conclusion of treatment or 8 years after death, and all other records 8 years after the conclusion of treatment or death. Sometimes medical records can be retained indefinitely if there is legitimate reason to do so.

Right to be informed

You have a Right to be informed about the collection and use of your personal data. The information on this Privacy notice explains what Physical Edge Ltd collects from you, why it is being collected and how it is used. If you have any further questions please contact the data protection officer.

Physical Edge Ltd will email you to confirm when your Personal Data has been processed. At this point you have a Right of access to your Personal Data and supplementary information.

You have right to request your personal data be erased.

Physical Edge Ltd holds medical records about your treatment. We follow the recommendations made by the Chartered Society of Physiotherapy when retaining medical records for medicolegal reasons. Right to restrict processing

You have a right to restrict the processing of your personal data. Upon receiving such a request Physical Edge Ltd will automatically restrict processing of your personal data until you receive a reply from us. You will always be informed before any restriction is lifted.

Right to object

You have a right to object to processing of your personal data when you make first contact with Physical Edge Ltd. Physical Edge Ltd must process your Personal Data, as a Healthcare company, however if you object to processing of your data please inform your Physical Edge team member without delay. Your treatment at Physical Edge Ltd will cease upon receiving your request.

Rights related to automated decision making including profiling

Physical Edge Ltd does not perform automated decision making or profiling.

You can make a request for any of your Rights, listed above, verbally or in writing to the Data Protection Officer. You can expect a reply within 28 days, free of charge, and in the same format as your request. A “reasonable fee” could be charged if your request is manifestly unfounded or excessive, particularly if it is repetitive. The fee will be based on the administrative cost of providing the information. For complex or numerous requests it may take a further 2 months before you receive a reply.

We may ask for Identification in the form of a Passport or Drivers license and possibly a utility bill. We would notify you of this requirement without delay and will only comply with your request once we have this information.

If we refuse your request we will notify you within 28 days and provide you with a reason why we are not taking action. You have a right to make a complaint to the ICO (Information Commissioner’s Office https://ico.org.uk/concerns/) and seek to enforce your right though a judicial remedy.

If your personal data has already been disclosed to other Persons/ companies who are authorised to process your data, Physical Edge Ltd will notify them of any changes made through your request, unless this proves impossible or involves disproportionate effort. We will inform you who we notify upon your request.
